Case Management
Our In-house Case Management Program is designed to have the flexibility of being tailored to each individual based on their needs. Broken down into three phases, our pathway of measurable success outlines a start and a finish for each individual willing to learn how to gain their independence, voice and skills to leave a positive footprint.
In-House Services
- Thorough Basic Needs Assessment
- Referrals & Navigation to Wrap Around Service Agencies needed
- Obtaining Photo ID, Certified Birth Certificate, Social Security Card, DD214 Form
- Bus tickets to various appointments
- OHP enrollment assistance / Notary
- Computer assistance with applying for food stamps, building resumes, housing applications and more
- Employment / Supportive Housing Support
Adult Mental Health Peer Support
Our certified Adult Mental Health Peer Support Specialists have lived experience of homelessness, extreme hardship, trauma, grief, depression and many more
In-House Services:
- Transportation to Mental Health appointments / Get prescriptions
- Support for those struggling with depression and social anxiety
- Support for those seeking to discover joy triggers, strengths and talents
- Art / Activity Room with supplies to draw/ paint / color
- Support with wellness activities including but not limited to walks on the beach, hikes, beach clean-ups, and various other physical activities
- Advocacy
Ticket To Home Program
Our Ticket to Home program is designed for people who are houseless and want to go back to their family in another state.
HOW THE PROGRAM WORKS
- Bus / train tickets are only available within the continental US. The destination location price will determine whether we provide a bus ticket or a train ticket. We do not pay for plane or boat tickets.
- Families must make contact with CSS Staff and provide a valid contact phone number and release address of the person seeking transportation assistance.
- Bus / train tickets are only available once in a lifetime per person. No exceptions.
